Our office accepts Cash, Checks, Visa, MasterCard, Discover, American Express and Care Credit. All treatment totals/estimated co-payments are due at time of service. Payment arrangements are also available. Please contact our office for more information. We are here to help you!
Insurance
In an effort to keep rising costs down, we participate with several insurance companies and networks. If you have changes to your insurance, please contact our office as soon as possible. We are in network with the following insurance companies:

- Anthem Dental Blue 100/200/300
- ASR
- Delta Dental Premier
- Humana
- Cigna PPO Radius
- Indiana Medicaid
- Children’s Special Healthcare Services
We are also providers for the following Insurance Networks: Connection Dental, Dentemax, Dental Benefits Providers, Stratose, Dental Network of America (DNOA) and Maverest DWP.
We always encourage you to call your insurance company to ask if we are an “in network” provider on your specific plan. Because we understand that dental appointments can become costly, we estimate your co-pay as accurately as possible. Co-pays are due at time of treatment. Affordable payment arrangements are available if necessary. Please speak with us prior to your appointment to set one up.
